Course Description: This session will focus on implementation of outcome measures through the use of a knowledge translation (KT) framework within a large healthcare organization. Examples across settings of care will be utilized to illustrate successful practice change by engaging key stakeholders, addressing barriers to implementation, developing KT tools, and using data to inform outcomes.
